In the ward, when I was meet with the patient who is facing dementia and as a result, he
was not able to take decision because of low mental health. Further, he is also suffering
from high BP and suffer from different personal issues as well. Such that he is not living
with his family and his son also did not take proper care of him. As a result, his mood
also swings and also aggressive in nature.

In this situation, as a health care provider I provide person-centred care in such a way
i.e. I treat that old man with dignity and respect which means that patient also feel that
there are someone who provide us proper care and treatment. In the case of old man, he
felt so lonely and that is why, his mood also changes, that is why, to provide proper
person-centred care, I make sure that I treat him with proper respect and dignity. On the
other side, in order to deliver consistent person-centred care, I also coordinate other
health and social care providers so that they easily rebuild the understanding in old man
and also receive proper care which they need. As a result, I adopt to make care plan of
that old man and also share with doctors in order to make sure that I am on right
direction. In addition to this, I also adopt standardized approach which can affect their
ability to recover and manage the condition and provide better quality of life in the
