INTRODUCTION
The present study will describe the two different situation through which a nurse provide
a person-centered care that helps to make patient comfortable within the place. Further, study
will describe own skills and abilities by using SWOT analysis and after analysing weaknesses,
study describe developmental plan that helps to overcome weakness.

In the ward, when I was meet with the patient who is facing dementia and as a result, he
was not able to take decision because of low mental health. Further, he is also suffering
from high BP and suffer from different personal issues as well. Such that he is not living
with his family and his son also did not take proper care of him. As a result, his mood
also swings and also aggressive in nature.
Provide evidence of how you have provided person centered care taking into
consideration the key principles of person- centered care. 1. Affording people dignity,
compassion and respect. 2. Offering coordinated care, support or treatment. 3. Offering
personalised care, support or treatment.
In this situation, as a health care provider I provide person-centred care in such a way
i.e. I treat that old man with dignity and respect which means that patient also feel that
there are someone who provide us proper care and treatment. In the case of old man, he
felt so lonely and that is why, his mood also changes, that is why, to provide proper
person-centred care, I make sure that I treat him with proper respect and dignity. On the
other side, in order to deliver consistent person-centred care, I also coordinate other
health and social care providers so that they easily rebuild the understanding in old man
and also receive proper care which they need. As a result, I adopt to make care plan of
that old man and also share with doctors in order to make sure that I am on right
direction. In addition to this, I also adopt standardized approach which can affect their
ability to recover and manage the condition and provide better quality of life in the

hospital.

In order to make old man comfortable within hospital, I make a list of care plan which
he actually needs and due to his aggressive nature, I always talk with care and love so
that he may share their grief. In this way, I get an attention and set a positive mood for
interaction so that he may talk. After that I keep asking question in simple manner so
that it will help me to answer in simple way. By performing this activity, I make sure
that I follow all the principle of care so that the person is not irritated. In that mean
time, I take medicine and feel comfortable in the area as well, but I cannot ask about
his family member, because it make old man aggressive as well.
Scenario 2: Give a brief description of the patient or service user or individual in
need that you have provided person- centered care to
in the general ward I meet with a old lad name Malcom who is 27 year old met
with a car accident, she had undergo a surgery for both arms and also suffered a
severe injuries at the time of crash. Therefore, she needs to remain in the
hospitals for about 2 week and as a result, a lot of amount will going to be spend
for her treatment. In the same time, I noticed that se consult with her doctor that
she did not want to stay in a bed as a patient, rather than this, she wants to enjoy
and reading books, interact with people so that she may enhance her knowledge. I
like this thing because she has a positive nature and ability to cope up with
difficult situation. But in the same time, I noticed she is aggressive in her nurse
because he did not provide person-centred care.
