INTRODUCTION
Integrated care can be defied as a kind of concern that can help in improving experience
of patients, and achieve high efficiency and quality of care from local health and social delivery
system (Loskutova and et. al., 2018). Integrated health and social care can help in providing
appropriate and ultimate care and support to service users which further helps in ensuring their
healthiness and wellbeing. For provision of integrated care and support local are care and
resources plays a vital role as it helps in development of an accurate care plan for providing
improved care to service user. This assignment will focus upon health, care and support services
available in Lambeth council.

Local community resources and provision that supports integrated care planning/working
Local community resources are kind of resources that served to a local or a particular
geographical area or to a group of people living in that particular area. Local area to be discussed
in this essay is London Borough of Lambeth and map of London Borough of Lambeth has been
shown below:

According to the case study Johnson is a 65 years old man who has recently moved to
Britain from St Lucia a couple of years back. He has no family and has weak left side because of
stroke and cannot walk without stick. He was also detected with early stage vascular dementia

due to which he forgets. Sometimes he even forget to take his medication of blood pressure ands
other medications. Due to this, his neighbours called for social services in London Borough of
Lambeth by expressing their concern that he is not able to take care of himself and a strong
unpleasant smell is coming from his flat.
Due to this condition of Mr Jhonson he has multiple needs and as a result he require
multidisciplinary teams. In order to develop an integrated care plan for Mr Jhonson a
multidisciplinary care planning is required to be done. Social services of London Borough of
Lambeth rely on number of partners throughout their council. Some of the main local community
resources that will be supporting Mr Jonson and his health are:
Adult social care and health centre which is a non-profitable organization that will be
providing required medical attention and care to Mr Jhonson for effective treatment of early-
stage dementia and of high blood pressure. They will also be providing care and support to
him so that desire of having weak left side he can move properly without any risk of falling
down. If Mr Jhonson do not want to leave his house, then this centre can also provide 24*7
carer to him who will be providing support to him, help him in daily routine activities,
provide him medicines from time to time. This can be done though carers assessment and
care plan. This will help Mr Jonson to enhance his physical and mental health, participate in
social activities, strengthen relationship with others,
Community development for older people which is a society that take care of older people
who cannot look after themselves (About Lambeth Council, 2021). They develop a care plan
for older people who require their support and care in broader sense. This society can
collaborate with community development for disabled people society So that Mr. Jhonson
can learn new skills and ways through which he can walk properly, participate in daily
routine activities
These support and integrated care planning can help social services to take care of Mr
Johonson and work together for enhancing his overall physical and mental health.
Local unmet need related to healthcare provision in London Borough of Lambeth area
Unmet needs are difficult to define and even harder to measure but it is extremely
important to identify and measure unmet needs of services users especially related to healthcare
provision. Local authorities involved in care plan for a service user are required to identify and
measure unmet requirements and needs of service user in order to provide them with effective
