INTRODUCTION
Collaborative working refers to bringing together a group of professionals from different
health and social care orientation, where they could share their knowledge and expertise to attain
a common goal. On the other hand, care network is a group of individuals responsible for
providing health and social care services to individuals (Ivbijaro and et. al., 2015). The report
below is based on collaborative working with this network in context with providing effective
medical and social assistance to individuals.
PROJECT 1
Collaborative working with James
Individuals suffering from depression and anxiety are required to be dealt with utmost
care and delicacy as they are prone to their condition and quite habitual of leading their lifestyles
in a negative manner. To develop a prominent professional relationship with James, it is very
necessary to ensure his collaboration. A proper communication channel would be established by
me where I would be hearing James regarding reasons for his behaviour, which would help in
establishing a relationship of faith and trust, which is important for working together. Moreover,
influencing James and reasoning with him would be two important methods that would be
effectively applied in this scenario, so as to effectively collaborate in achieving better health
standards.
Activities to help James build his resilience
There are several activities which would be undertaken in context with helping James
build his resilience. These tasks are discussed as under: Counselling: The very first task would be to provide essential counselling related to
several issues faced by James. This would include helping the individual in nurturing all
the positives about his personality (Black and Gallan, 2015). Moreover, sessions of
advising would also be a major part of counselling where James would be advised of
following several positive activities like exercising and meditations in order to improve
his condition. Goal-Setting: Taking action is utmost important in order to ensure that a proper pathway

would be set along with several time scales for James to work upon each of his issue and
enhance his health standards accordingly.
Helping in building up James' current network and its expansion
As reflected in the case study, James rarely go out and has stopped meeting his friends or
family. This calls for building up his network as well as expanding it further. In order to help him
do so, he would be taken to social support group, such as the ones driven by NHS, where
individuals suffering from anxiety and depression come together are provided guidance by other
people which influences them positively. Thus, James being a part of such social groups would
develop a quite positive perception in context of him not being along with this condition. Thus,
his confidence would boost up regarding taking effective actions about his condition as well as
meeting new people would encourage him to go out with them and meet new individuals. As for
expansion, James' social circle would be expanded by making him part of several social and
motivational events happening around the city (Gupta and et. al., 2016).
Involving James in the community
In order to enhance resilience, it is highly necessary to involve James into the
community, where he could do activities which interests him as well as expand his social
connections. In order to do so, an effective interest list is developed as under, so as to help in
gaining knowledge about activities which are being preferred by James and how much
involvement as well as curiosity is within the individual regarding these tasks. Thus, the interest
list for James is mentioned below:

in context with the degree of interest. Doing so would help him being a part of the community as
he himself would be performing these activities and meeting new people as well as be an active
member of the society.

Therapist: The very first medical professional which would be linked to James would be
a therapist. They are professionals who undertake several therapy sessions to ensure a
better mental health in individuals. Thus, they would help in treating anxiety and
depression of James. Psychiatrist: Another effective medical professional for James is psychiatrist, who would
prescribe medicines to the individual which would help in treating his anxiety and
depression. Professional Doctors: As described, James is prone to consuming alcohol and marijuana.
Such high level of dependency requires medical assistance to help overcome the
addiction (Minkman, 2016).
Local Services: Rehabilitation Centre: This service would be beneficial for James in dealing with his
dependency on substances like alcohol as well as marijuana. Social Groups: This service would help the individual in enhancing his social circle as
well as would provide motivation which would contribute in a better mental health.

assistance which could help James in bettering his condition. For the same, I would refer him to
see a psychiatrist. Further, I could not medically help James in leaving his addiction related to
alcohol and marijuana. Thus, for the same I would introducing him to rehabilitation centre,
which would provide this individual with professional treatment which surely would contribute
in helping him overcoming his addiction and lead a healthy lifestyle.
Review of effectiveness of services and identification of gaps
Medical and social services are required to be reviewed and monitor so as to ensure
whether the aspects associated with the treatment is being provided as per the desired level. In
context with review the effectiveness of services and identify gaps, there are several methods,
which would be applied that are mentioned below: Medical Reports: These reports would allow a detailed and quantifiable measurement of
health levels of James. Thus, in case medical reports from rehabilitation centre fail to
show improvement in blood samples and biliary levels, he would be required to be
provided with external medical assistance under heavy supervision of a senior doctor
(Foster and et. al., 2016). Therapists/Psychiatrist Reports: These would be showing the mental health condition of
James. Thus, the gaps would be identified in context with his optimism and will to
improve. In case there is a requirement of additional services, he would be then
connected to different and better social groups in context with inducing optimism in his
vision of life.
Ways to negotiate, advocate and assist James with his recovery goals

negotiated in relation to his recovery goals. These ways and methods are described as under: Communication: One of the prominent ways through which James could be negotiated

Addressing difficulties experienced by James
In case James experiences any sort of difficulty in his road to treatment, then there are
several measures which would be adopted by me. For instance, a detailed and continuous
feedback would be provided by James, where his condition and difficulties would be assessed
(Weiss and et. al., 2017). Thus, by active listening, problems would be identified which would be
communicated to his care providers along with specific instructions regarding areas that require
major focus. In case they fail to comply with the same, new support groups and care networks
would be approached to assure effectiveness.
CONCLUSION
Thus, it is concluded, that collaboratively working with care workers is highly essential
to improve mental and physical health of people suffering from anxiety and depression. It is very
important to implement several techniques like checklists and other social and medical methods
which are associated with promoting healthier lifestyle. Lastly, a care network must be
approached and worked together with, that would be responsible for providing the best care and
medical assistance that help individuals with inappropriate mental health and substance addiction
to lead a better life ahead.
