1. Introduction
The incident that caused problem in the Truss construction shop can be differentiated in various
ways as there are different problem that can be identified within the following case. The frontline
problem that can bee seen in the case is the failure of truss load test. Secondly the failure of load
test resulted in compromise of safety measures of hoist operator supporting the test which
another issue. The third problem that has be measure is the presence of flawed management and
engineering process that focused on production and low threshold load test of trusses to meet
demands. The fourth problem is that the sale department fears that after such incident the
company will be unable to meet demands affecting the company’s revenue and the last problem
being that the information of such incident was leaked by a production line manager to the press
which will question the authenticity of the company in maintaining safety of worker and in
production of good quality trusses.
2. Explanation of the Issue
In the introductory section problems have been identified and it can be said that the case has
various problems involved. The problem will be critically evaluated and explained in this section
the first problem being the failure of truss load test. It has been seen that truss were made for
bearing load under commercial high load requirements but still the following failed to deliver
successful results in test. The company has heavily invested in the manufacturing of cost
efficient truss that is to be an frontline product of the company. The failure also resulted in
sending shockwaves over the quality of manufacturing which the company is conducting for the
production of these trusses (McPeck, 2016). The second problem was that the accident caused
fatal injuries to hoist operator supporting the test the safety officer in this regard said that team
conducted heavy testing which caused such accident but the question stand on the safety standard

implied by the officer in the process of such heavy testing. The third problem has been over the
quality of production of these truss being cost affect the question arises on whether there has
been quality compromise on the truss because the management and the engineers have been
blamed over not focusing on quality of the truss and only producing to meet the demands of the
customers which has had an effect of the quality of the product which has resulted in the failure
of the product to be successful in the test. The sale department also fears that of the company is
not able to produce good quality product and is not able to meet the demands of the customers it
will have to face problems which will directly affect the revenue of the company. After so many
internal problems within the organization it also faced problem in which external pressure was
built on the company due to leakage of detail by zn production line worker which put the
company’s operations and production in the line of questions in which workplace safety and
others were questioned creating an external pressure over the firm (Jackson, 2015).
3. Analysis of the Information
From the information gathered through the case it has become very evident that there was a
series of mistakes which were committed by the company and which interested in consequence
of such accident. Critically evaluating the information gathered it can be said that firm has meet
the demands of the customer for which it has to regularly produce truss which are to take good
amount of load. Although the sales team may have created the pressure over the management
and the production engineers to increase production rates although this could be reason of the
decrease in quality of the product. Although the management was keen on providing cost
effective truss which means cost will be optimized over the production of the truss and this also
can be a possible reason of the compromise over the quality of the truss. Quality compromise
would have lead to the failure of the truss in the ting process causing such fatal incidents within

the company. Analyzing the above information it can also be said that Safety Officer was not
able to monitor the such risk situations due to which proper safety measures was not taken
causing in fatal injuries to hoist operator supporting the test. Overall through the information
gathered through the case it can be said that there are various factor and management failures
which have lead to such mishap within the company one of the prominent examples is that the
production line worker has leaked important information in relation to the accident which has
increased the overall problems of the company by creating an external pressure over the firm in a
very prominent manner (Cottrell, 2017). It is to be mentioned that the company has invested a lot
and has expectations from this product, failure of such product to deliver is a huge setback to the
company, and ts will have to find better mitigation ways to achieve better results from the
product. In the present scenario no management and team is taking responsibility but the
company will have to take responsibility and will have to mitigate the issues so that the firm
performs well in the near future (McPeck, 2016).
4. Consideration of alternative viewpoints and conclusions
Now there are various viewpoints from the Safety officers, management, production engineers,
and sales team. In accordance to all the viewpoints, it can be seen that there is low amount of
communication and coordination within the company which would help the company to render
high quality service. It can be said that through the viewpoint of management and engineers there
is peer pressure over temp to meet the demands of the customer as the sale team has reported that
the company will suffer through this process. Whereas the employees at the enegiring shop
blamed the production engineer for not meet the requirements of “commercial-high”. Through
the viewpoint of safety office the team conducted heavy testng due to which such failure
occured. Hence there were various viewpoints based on which it can be said that there are flaws

in each of this department and that the departments have no communicated well to ensure that
such problem occured. Hence the company will first have to ensure that there is effective
communication between these departments to ensure that the demands and quality measure to
effectively increase the overall performance of the firm (Paul, 2018).
5. Conclusions and Recommendations
Concluding in the light of above context it can be said that the truss construction shop should
establish effective communication between the department to effectively communicate the
demand and quality of the product and this will protect the company to ignore such flaws in the
future. It is recommended that the company follow effective test procedure which standard and
will make a safe testing environment to ignore fatal injuries of worker for testing. The company
should follow the cost optimization policy but should not compromise with the product quality
and should make standard products irrespective of the demand. The sale team should ensure that
early orders are made to ensure the company has time for production of the truss ensuring the
quality of the products. The employees might be addressed that no one should lead internal
details to outside press or media as confidentiality is to be maintained.
