The incident that caused problem in the Truss construction shop can be differentiated in various
ways as there are different problem that can be identified within the following case. The frontline
problem that can bee seen in the case is the failure of truss load test. Secondly the failure of load
test resulted in compromise of safety measures of hoist operator supporting the test which
another issue. The third problem that has be measure is the presence of flawed management and
engineering process that focused on production and low threshold load test of trusses to meet
demands. The fourth problem is that the sale department fears that after such incident the
company will be unable to meet demands affecting the company’s revenue and the last problem
being that the information of such incident was leaked by a production line manager to the press
which will question the authenticity of the company in maintaining safety of worker and in
production of good quality trusses.
2. Explanation of the Issue
In the introductory section problems have been identified and it can be said that the case has
various problems involved. The problem will be critically evaluated and explained in this section
the first problem being the failure of truss load test. It has been seen that truss were made for
bearing load under commercial high load requirements but still the following failed to deliver
successful results in test. The company has heavily invested in the manufacturing of cost
efficient truss that is to be an frontline product of the company. The failure also resulted in
sending shockwaves over the quality of manufacturing which the company is conducting for the
production of these trusses (McPeck, 2016). The second problem was that the accident caused
fatal injuries to hoist operator supporting the test the safety officer in this regard said that team
conducted heavy testing which caused such accident but the question stand on the safety standard

been quality compromise on the truss because the management and the engineers have been
blamed over not focusing on quality of the truss and only producing to meet the demands of the

of the product to be successful in the test. The sale department also fears that of the company is
not able to produce good quality product and is not able to meet the demands of the customers it
will have to face problems which will directly affect the revenue of the company. After so many
internal problems within the organization it also faced problem in which external pressure was
built on the company due to leakage of detail by zn production line worker which put the
company’s operations and production in the line of questions in which workplace safety and
others were questioned creating an external pressure over the firm (Jackson, 2015).

From the information gathered through the case it has become very evident that there was a
series of mistakes which were committed by the company and which interested in consequence

and the production engineers to increase production rates although this could be reason of the
