Introduction
This assignment consists of three sections. All the three sections are based on the case study of
the Working in Partnership of Mr. Ian and is going to explain the philosophy of working in
partnerships, different models involved in the process of partnership, its working and its
outcome. This task is also going to deal with the potential barriers, that comes in when the
Health and Social care organizations gives effort in partnerships.
Section A
This section of the assignment is going to give explanation about the attitude of working in
partnerships and its effectiveness of partnership relationships within Health and Social care.
Explain the philosophy of working in partnership
When the Health and Social care employees starts functioning in collaboration and get busy in
the work related to the health and social care, then they are believed to work at different levels of
partnership (Berge, 2018). Moreover, they play a significant role, while working in partnerships
and moreover the performance and potential of all the individuals get correlated with each other
in terms of its development and growth. The different levels are given below –
Shared vision and attitudes – According to the provided case study, Mr. Ian who had witnessed
physical, sexual and emotional abuse during childhood and as a result of which he has become a
victim of depression, self harm and paranoia. In the later years, he has became physically
disabled. He was taken to a number of health care units for improvement in his health but despite
much of the efforts of the employees of the health care units, they failed to develop a good
working relationship with him. Mr. Ian’s verbally and occasionally physically aggressive
behaviour was the main reason behind the non establishment of any good relationship (Horwath,
2018). This failed relationship with the employees of the health care units has resulted in lack of
care for Mr. Ian, which aggravated his health condition. Thus, the shared vision and attitudes
were not good between the employees of the health care units and Mr. Ian.
Interpersonal levels – In this level, people who are seriously ill are incorporated. They are not
being able to converse properly with the members of the health care units about their needs and
expectations. As a result, they have to supported by the staffs of the health care units in such a
way that their needs and requirements gets fulfilled. However, the staffs of all the health care

of good or bad relations among the employees, care workers and service users in the creation of
efficient interpersonal relations.

Local Clinical Commissioning Group – This part includes the heath care centres working
locally for the treatment of Mr. Ian. The health care centres are given below –
Local Authority – This centre used to provide daily monitoring and support for the
betterment of Mr. Ian. However, his case was moved to Local Authority Long Term Team
due to lack of improvement (Cheraghi‐Sohi et al.,2018.p. 20).
Local Authority Long Term Team – Mr. Ian’s case was transferred here from LA after
close monitoring of no sign of improvement in his health. From here, his case was further
moved to the Mental Health Review and Reablement Team.
Mental Health Review and Reablement Team – Despite much of efforts of the employees
of this centre, no working relationship was developed between the employees of the centre
and Mr. Ian.
Risk Enablement Panel – The doctors and the workers assessed Mr. Ian of having mental

handling a case like that of Mr. Ian with ease and efficiency. A smooth functioning of the
organization can be assured with informed staff that can perform with confidence, proving to
be an additional help to the patient along with the institutes (Walker et al., 2018).
Allocating the roles among the staff is another important factor that is to be undertaken in
this context in order to avoid any unnecessary confusion among the individuals regarding
their roles in guiding the patients back to health (Scantlebury et al., 2018). Staffs working
with the patient are to be aware of the duties and services they are entitled to provide on their
part. This will also help them perform the same without any related hassles.
Roles based on qualification is one of the major strategies that can be put to use in this case,
referring to Mr. Ian’s caretakers that hardly had the required training to assist and work with
the patient. This also proved to be an obstruction on the part of the practitioners to deliver
relevant feedback to the organizations related to the case. Trained professionals can be hired
