Effective care in health and social care is to providing one of the best care which is
deliver to patient in order to get better health of patient. In relation to deliver effective care to
patient, there is need to take different assessment which can able to provide better care to patient.
Aim of this report is to develop knowledge and understanding about the different form of
assessment in relation to develop review and planning skill during conducting assessment. In this
report, there is a discussion about the purpose of different assessment models along with
assessment tool which are there used by an organization. There is also discussion about the way
to use person centred care in relation to participate in assessment process. There is also
understanding how to manage different assessment outcome by developing action plan. There is
also discussion about the process in relation to access staff and its impact on individual or
patients family (Taylor, 2020).

Explanation of the purpose of different assessment models
Health assessment refers to general understanding about the state of individual health
which includes physiological, psychological, mental and sexual well being. This is the one which
enable individual to proactive stance towards health and screening of certain diseases. There are
different purpose of health assessment which provide high level of benefit regarding health and
social care. Assessment refers to collection of medical test and different examination which are
there performed on a person to find any disease or any change in physiological or chemical
change in body. This one is requested by any physician in order to evaluate fitness of any
individual for performing better job regarding providing better health and social care. Each and
every person need to have better health and state of mind which need to be careful regarding
health. Where it can be see that due to regular health assessment, there is deduction in 40% of
death from cancer and about 80% reduction on type 2 diabetes patient. This is the one which
can't be prevented buy regular health assessment. This one is able to provide the information
related with individuals health and social care (Black, 2017).
Health assessment is able to provide guidance along with management strategy in relation
to provide healthy diet along with type of regular exercises which can provide benefits to patient.

This is also able to prevent the onset of chronic disease. During the process of health assessment
there are different type of screening test which has to apply on specific individual or group to
find out medical issues, their management and medication is can provide health to invisible
regarding better health. Health assessment is highly need to establish whether any individual
need for prevention or management regarding their quality of life for overall health. Purpose of
health assessment therefore providing better health and social care to patient in relation to
prevent from any chronic disease onset (Axelrod, 2018).
Purpose of assessment tool in Healthcare
Assessment tool refers to technique which are generally used to measure academic ability
skills and influence recording any topic. It is the one which measure the progress towards
proficiency in a specific area. In health assessment tool, there are standardized system which are
able to help to identify specific condition in order to provide fair approach in response. Health
assessment are generally use for screening and find out the medical condition which need to be
care about patient or individual. There are different types of assessment for better health and
social care such as cough assessment, most fall risk, stroke, scale CAGE assessment, waterlow or
Barden scale for assessing ulcer, risk activities of daily living scale. These are some of the
assessment tools which are generally used in Healthcare premises and also used by Sandy Bay.
Main purpose of this assessment only has to find out the issues which are there in individual in
relation to provide better health and social care (Fulmer, 2018).
This is the one which is able to provide the opportunity to get diagnose in earlier stage of
any diseases. This can provide effective Health Care Management and this assessment provide
some time to individual for better quality of life. This also helps to to free medication and to
formulate different management strategy and plan in relation to get healthy and get rid of it. In
Sandy Bay, there is generally cough assessment tool and daily activities of living scale prior to
get information regarding health of an individual for their better health and social care.
