INTRODUCTIONHealth and social care refers to a term which relates to a services that are provided byhealth and social care providers to their patients. It is a term which refers the whole of the healthcare provisions and provides the treatments for ill health and medical conditions within thehospitals, community and health care centres (Read and et. al., 2017). This report is based uponthe individual's assessment in a health and social care settings which includes the range andpurpose of different forms of assessment and how partnerships supports the assessment process.Present report also discusses about the early assessment, active participation of an individual inshaping the assessment process also developing a care or support plan in collaboration withindividual that meets their needs. This report also includes developing other's understanding offunctions of rage of assessment tools and understanding how assessment may have positive andnegative impact on other's and their families. This report also includes the reviewing, evaluatingthe outcomes and developing an action plan in order to address the findings. LO 11.1 Comparing and contrasting different forms of assessmentThere are different forms of assessment and all these assessment serve with differentpurpose during and after instructions. Main purpose ofBehaviouralassessment is todescribe, predict,explain andsometimescorrect thebehaviour.Its purpose is todevelop acomplex mentalresponse towardsthe work of anindividual The main purposeof person centredplanning is that tohelp the person inall aspects of lifeso it can have aquality life. 1.2 How partnership work can support assessment processPartnership work can positively support the assessment process as partnering withpatients, friends, families and various care professionals like social workers can give betterunderstanding of what type of a care an individual needs and also to ensure that these needs areeffectively met. Working in partnership can provide a lot of information about the individual andthus accordingly organisation would be able to offer better service and build better picture of theservice users (Schulz and Eden, 2016). Partnership work will highlight the principles whichdetermine good practices such as Rights, Privacy, Emotional needs, confidentiality, valuingpeople, etc. Working in partnerships with others can ensure that service users who are using thehealth and social care service should be benefited.
