FOR RESEARCH INVOLVING HUMAN PARTICIPANTS
Before completing this form student should familiarise themselves with general
Research Ethics and Ethical Code of Conduct
Please pay particular attention to the ethical principles of respect and responsibility.
Note that research solely involving analysis of existing data (secondary analysis)
should complete separate forms.
HOW TO COMPLETE & SUBMIT THE APPLICATION
1. Complete this application form electronically, fully and accurately.
2. Type your name and ‘student’s signature in the designated area.
3. Include copies of all necessary attachments.
4. Email your supervisor /lecturer the completed application and all attachments as
ONE DOCUMENT. INDICATE ‘ETHICS SUBMISSION’ IN THE SUBJECT
FIELD OF THIS EMAIL so your supervisor /lecturer can readily identity its
content. Your supervisor will then look over your application.
5. When your application demonstrates good ethical protocol your supervisor will
type in his/her name in the ‘supervisor’s signature’. Your supervisor /Lecturer
will let you know the outcome of your application. Recruitment and data
collection are NOT to commence until your LSME ethics application has been
approved, along with other research ethics approvals that may be necessary.

1. A copy of the invitation letter that you intend giving to potential participants.

2. A copy of the consent form that you intend giving to participants.
OTHER ATTACHMENTS AS APPROPRIATE
A copy of questionnaire(s) that you intend to use. Please note that copies of
copyrighted (or pre-validated) questionnaires do NOT need to be attached to
this application. Only provide copies of questionnaires (i.e. ones you have
written or made yourself). If you are using pre-validated questionnaires and
other copyrighted stimuli (e.g. visual material) make sure that these are
suitable for the age group of your intended participants.
A copy of the kinds of interview questions you intend to ask participants.
A copy of ethical clearance or approval letter from an external organisation if
you need one, and have one (e.g. Argos, Apple, schools etc.). Note that your
LSME ethics application can be submitted before ethical approval is obtained
from another organisation. If you need it, but don’t yet have ethical clearance
from an external organisation, please let your supervisor know when you have
received it.

No there is not any consequences with the participants. All are cooperative in
nature and project has been done smoothly

due to different opinion.
