1. Executive Summary
The paper mainly focuses on the creation of nursing handover tool in order to
handover patients from ICU to other wards or hospitals. It is identified that earlier
the healthcare industries utilizes manual reporting system during handover of
the patients from ICU to other wards or hospitals. However, due to the utilization
of manual reporting system, numbers of issues as well as challenges occur
during handover due to misplacement of some information or data. In order to
avoid such circumstances, the organization wants to develop nursing handover
tool with which patients can be handover easily as well as efficiently. Creating
nursing handover tool not only improves patient’s security during handover but
also helps in reducing errors that occur due to misplacement of information and
data. The project sponsor is the CEO of the healthcare who generally provides
the project amount that is needed for completing the project. It is identified that
the project generally needs 91 days as well as budget of around $101, 800.00 in
order to complete the project successfully. In order to execute the project
successfully, the risks that are associated with the project are identified. It is
analyzed that the project faces issues due to inappropriate budget estimation,
schedule slippage as well as due to inappropriate security related strategies. The
paper identifies steps that are helpful in resolving project related challenges as
well as issues. It is analyzed that in order to resolve security related issues it is
very much important to have proper authentication. In addition to this, the paper
also reflects on reporting processes, communication strategies as well as risk
management plan in order to execute the project successfully within the
estimated time and budget.

2. Overview
2.1. Project Title
The project title is “Creating a nursing handover tool” for
transferring patients from ICU to other wards and other hospitals.
2.2. Project Background
It is identified that earlier the hospitals utilizes manual system for
recording the details of the patients, which they transfers during shifting
patients from other wards or hospitals. However, maintaining a manual
system while handing over the patients generates number of risks and
challenges. If any data or information is misplaced then, it can threaten
the life of the patients who are in the critical conditions (Gibbons et al.
2016). Therefore, in order to reduce this issue and for improving safety as
well as efficiency of patients handover the healthcare organizations wants
to create a nursing handover tool. With the development of nursing
handover tool, the healthcare organization will be able to improve the
safety of the patients. In addition to this, they will be able to achieve
proper as well as efficient communication of various clinical information at
any time when the patient care is generally transferred.
3. Scope
3.1. Project Problem Statement & Project Goal
The healthcare organizations earlier utilizes manual record in order
to transfer professional responsibility as well as accountability for all the
aspects of patients care to another ward or hospital. However, utilization
of manual system does not provide proper security as well as safety as the
data can be misplaced or repeated twice. As a result, the healthcare
organizations are facing challenges in maintain the safety of the patients
after handover.
In order to reduce the challenges as well as issues that the
healthcare industry is facing due to the utilization of manual system, the
healthcare organization wants to create a nursing handover tool. It is
identified that with the help of the handover tool, the organization can be
able to transfer patients from ICU to general wards or other hospital quite
effectively. This is because all the information that are associated with the
patients are stored with the system and the information can be easily
accessed at any time irrespective of location and which assists in
maintaining the safety of the patients while handover.
