1. Background/Overview
As a support worker, part of your role is to facilitate community
integration and skills building. Developing the skills of people who
are disadvantaged will lead to empowerment an independence for
the person. Much of what we learn is from incidental learning, and
clients will learn from you as a support worker just through
observation. Each person’s skills building plan must be unique to
their individual needs. This assessment follows the CareShore
clients that narrate the course. Use the case studies to
demonstrate your knowledge of community participation and skills
development.
2. Brief
In completing this assessment, you will demonstrate your
understanding of how to contribute to ongoing skills development
using a strengths-based approach, as well as supporting
community participation and social inclusion.
This assessment has three (3) parts, each consisting of a number
of short answer questions.

1. Answer the following questions based upon the scenario below.
Peter, 45 years, from a metropolitan city.
Peter had a fall from his roof and sustained paraplegia and an acquired
brain injury. Since his accident Peter has had some further difficulties.
He will go into the community on his own in his wheelchair and get lost.
He has become prone to aggressive outbursts when he gets frustrated
at not being able to do things he once did. His wife, who had been the
primary carer, felt unable to cope any more on her own and arranged for
Peter to live in a Supported Living home managed by CareShore. Peter’s
dream is to get back into the workforce in some capacity, to play
basketball as he once did, to manage his emotions, to access the
community independently and not get lost, and to ultimately live back in
his home with his wife.
a. Imagine you are Peter’s support worker, and are assisting Peter to
identify some of his skills building and community participation goals.
What might these be? (20-50 words)
As a support worker of peter, I would like help him to improve the
existing skills which he carries and support him to build an ability to
demonstrate new skills.

b. Imagine you are working with Peter to identify some services and
networks in the community for a person with Peter’s needs and
interests. Conduct some research on at least two (2) services that
would be of benefit to Peter and complete the required information in
the table below.

c. How would you help Peter to identify any further interests, abilities and

Peter need assistance in finding his hidden ability and skills, I would help
him in identifying these interests and abilities by promoting inner
morale of peter. I will guide him with proper plan and help him to
accomplish small goals and objectives.
d. Peter may or may not be interested in doing the activities you have
identified. How would you present these different options to Peter for
him to make decisions about whether he is interested? (20-50 words)
The interest level of peter in doing these activities such as music
therapy which I have provide is quite low but I will encourage him to
make his own decision in choosing activities which he like to or
interested in.
e. Explain why it is important to involve Peter’s wife in your planning. (20-
50 words)
It is very important to involve peter's wife in the process because, every
person is somehow connected with the loveone so for peter his love one
is his wife which is emotionally and mentally connected with the peter.
And another reason is that, peter's wife have experience in taking care

of her husband before she arranged support work or caretaker home.
2. Read the scenario that follows on about Peter, and then answer the
questions that relate to it.
Peter has agreed to have a planning meeting around his community
participation needs and to formally agree upon some goals. Peter’s wife
attended the planning meeting also and is supportive of his choices and
keen to assist where she can.
a. Prior to a formal planning meeting occurring, identify what kind of
assessment the following professionals may conduct with Peter.
Professional What kind of assessment might they conduct
with Peter:
Psychologist Psychologist will allow peter to stable his
psychological needs and wants, they can solve half
of the problem of peter by their psychology
assessment.
Occupational
therapist
Occupational therapist might be very beneficial for
peter because there assessment will help him to get
therapy to recover any illness. These therapist allow
peter to develop, recover and improve with
everyday activities.
Speech
pathologist
Peter have fallen from great height which have
injured some of his vocal part, that is why is need to
have speech pathologist who will help him to
recover from diagnose of lack of proper speech and
ability to communicate.
Physiotherapist Physiotherapist will be very beneficial for peter as
they will help him to recover from injury with proper
exercise and fitness plan. And they even going to
