INTRODUCTION
Many entities use self appraisal-form in which an individual can easily improve their
performance in the entity. With the help of this form, an individual can easily find out its
responsibilities that must be full within the organisation. In order to this, worker get to know
about it performance, objective and can take necessary action towards the improvement. Present
report is based on NHS (National Health services) in which I am working as a nurse. An
employee having various roles and responsibilities that must be accomplish by them in effective
manner. In addition to this report, a nurse who is working in the NHS that develop her self-
appraisal form in which they concern with necessary improvements in their work life. (Self-

nurse having when she work in the healthcare institute. There are many ways in which they can
manage their work in more effective manner so that they can perform in their institute in a better
way.
