INTRODUCTION
There are some practice theme in health and social care which an clinical practitioner should
know so they will be able to conduct daily work in more effective manner (Buse, Martin and
Nettleton, 2018). Moreover, there are five practice theme i.e., patient focus, information focus, quality
improvement, staff focus and leadership. All these are interconnected and together provide framework
to look more closely at those things which makes differences to quality of care. Within the assistance
of PDP plan individual can develop these key within them and able to serve people in more effective
manner. Professional development plan is list of actionable steps which use for accomplishing career
goals (Mazzotti and et. al., 2018). In addition to this, professional development plan assist individual
insight into how they can reach career aspiration like earning new certification or finding mentor who
will advise.
TASK 2
P3 How the Practice Themes can be used to underpin reflection in learning and practice.
There are mainly five practice theme in health and social care i.e., patient focus, information
focus, quality improvement, staff focus and leadership. By learning all these key Themes individual
will be able to serve people in effective manner as it will result in gaining positive outcomes. In
simple term it can be said that, these five key themes assure quality performance and activities related
to heal and social care will be conduct in more appropriate way (Aldosemani, 2019). Here, each and
every indicator have some different meaning such as patient focus means how services are based on
requirement of patients, information focus means way to use information, quality improvements
means how standards are reviewed and attained, staff focus is related to staff development and
leadership means way to plan improvement efforts. In development of these entire professional
development plan play important role as it help in learning each and every skills in effective manner.
Thus, it will assist in gaining better future advantages as well as career development. In addition to
this, by focusing on these themes in effective manner individual will able to accomplish their goals
related to health and social care professional in effective manner. Because all these themes i.e., patient
focus, information focus, quality improvement, staff focus and leadership all together play important
role in gaining better future opportunities within effective manner.
P4 detail the links between the Practice Themes and other performance indicators.
Professional development plan is list of actionable steps which use for accomplishing career
goals (Adams, Dominelli and Payne, 2017). In addition to this, professional development plan assist
individual insight into how they can reach career aspiration like earning new certification or finding

mentor who will advise. There are several areas which required to be develop ny me for gaining better
future opportunities as an professional such as Quality care procedures, Communication skills,
Recording and Reporting, Developing diversity in nature, Quality support and Care as well as IT
technology. By improving these areas in effective manner i will be albe to perform my professional
activities in effective manner and accomplish my future goals. I want my future as an good health care
practicioners in which all these skills and knowledge play important role. So it is important for me to
develop these skills in effective manner and accomplish predetermined goals and objectives related to
my career.

CONCLUSION
After going through entire discussion it has been summarised that, there are five practice
themes within health care and it is important for individual working in this sector to learn all these in
effective manner. So they will be able to serve people in appropriate manner according to the
requirement. Thus, in development of this professional development plan will play important role as
with the assistance of all these they will be able to develop skills in more effective manner.
