INTRODUCTION
Case Management Process refers to the different types of phases which are adopted by
the case managers so that they are able to review the particular cases in the right manner and
ensure that they can solve the particular case in the right manner effectively and efficiently in the
right manner (Chabuk and et.al., 2016). This essay will focus on solving of the problem of a
specific client. In this essay, a detailed focus will be made on discussion of the way in which the
initial phase of the case management process can be used so that the problem of the client can be
resolved.
MAIN BODY
The Case Management Process has different types of phases which are used in order to
resolve the case properly (Franklin, Smith and Fowler, 2016). It has several phases which are all
very valuable in ensuring that the solution to the cases can be done. This will be quite helpful in
ensuring that the identification of the case can be done and the appropriate methods and
techniques can be used to resolve the case in the right manner. It is a process in which the
analysis of the case is undertaken step-by-step so that a proper solution to the case can be
provided effectively and efficiently in the right way.
Case- Peter is a client who has approached the NDIS for getting support in relation to the
problems which are being faced by him. Thus the use of Case Management Process will be made
so that the understanding of his case can be done in the right manner and the proper solutions are
provided so that the case can be resolved effectively and efficiently. As a NDIS Coordinator I
will need to make the use of specific approaches and techniques so that I am able to resolve the
problems and issues of my client Peter in the right way.
Approach to be considered for making initial contact with Peter-
There are various types of approaches which can be considered while making contact
with Peter. These are Problem Identification, Gathering and Assessing Information. Thus, I will
ensure that I make the use of Problem Identification approach. The reason that I will make the
use of Problem Identification Approach is that by making its use I will be able to identify the
problems of Peter which he is facing and provide a solution to them by studying his case very
closely in detail.
