INTRODUCTION'My Health Record' is the secure online synopsis of an individual health information andwhich is the project of The Commonwealth Government. The project funding has been approvedand in this report stakeholders of this project will identify and positioned with the help ofstakeholder mapping. The role names of stakeholders who will be provided access to the projectwill be identified and explanation of their interest will be provided. Questionnaire will beprovided to one of the stakeholder in order to obtain information regarding issues they facedregarding patient medical information.
