Patient: Mrs. Violet Kwong
1. Patient situation: From the current situation, it can be clearly ascertained that the patient
Mrs. Kwong was admitted in the hospital overnight and the lower BP, increased heart
rate and respiratory rate seem to be the major causes. The major cause for this present
state of Mrs. Kwong was the loneliness and depression that she was facing after her
husband had left and the fact that she is away from her friends and family who are in
Malaysia. The isolation that she is suffering from has also made her relationship with her
children superficial where she perceives that they are communicating with her only as a
facade. This has affected her mental state and the wound on her leg is also an increasing
concern for her but overall it can be said that the mental state in which she is in is more
complex than any physical evidences of deterioration in health which are not present in
her.
2. Collect cues/information:

Have you coped up from the incident of
your husband going away?
The reason that this question will be asked is
that the mental state of the person is very
difficult to be understood without proper
insight into the actual grief that they are facing.
Further, when the cognitive thinking of the
patient, the mental exertion that they are facing
and the behavioural state in which they are is
key in designing the care plan for the patients
(Xia & Li, 2018). For instance, this question
will help in understanding what the current
mental state of Mrs. Kwong is i.e. whether she
is still living in th past or has she accepted the
fact that her husband has went away and
therefore she has moved on. In both the
different cases, care plan would obviously
differ where in the former care plan would be

more retrospective and indoors as compared to
the care plan in second case which would
include more physical activity.
Why do you think your children are not
really concerned about you?
Patient has often found to be complaining
about her two children where she says that the
girl comes on weekends and just keeps talking
and then leaves without thinking to ask even
once about the actual heath of her mother. Son
on the other hand just talks on calls and hangs
up within few minutes. Now asking this
question will force her to seriously think about
the relationship that she actually shares with
her children and she might change her opinion
about them later on. It has been identified that
the support of children in the older age is
extremely necessary in identifying that
whether the patient is in a mentally stable
ground or not (Shvedko & et.al., 2018). This
will help in developing correct activities for
Mrs. Kwong where her interactive sessions
with the local children around the premises or
with other older ladies can be increased so that
the loneliness aspect decreases.
Do you really think that your health has
been affected or are you ill with any
serious concern?
This is the most critical question that can be
asked with the patient and there are chances
that this question can even offend the patient.
But, it is necessary to ask Mrs. Kwong this
question because she does not shows any
serious health consequence and the only
trouble that she is facing is mental or
emotional. If she is happier from her
surroundings or from the environment in
which she lives then it will get easier for her to
conform towards leading a better life.

Therefore, asking this question will help in
probing that whether there is need of any
practical i.e. operational treatment for her and
the care plan can be more psychologically
oriented rather than being practical thus
helping the patient (Campagne, 2019).
Are you at the state of mental peace? This is another question that probes the mental
peace rather than the physical evidence and in
this the patient themselves think about the
reasons that why they are unhappy. This is the
most effective retrospection where the patient
begins to search the reasons for their
unhappiness themselves thus identifying and
solving the reasons or causes themselves. This
can contribute effectively in the care plan
developed for Mrs. Kwong where she can
engage in mental exercises and activities as she
pleases based on the results that she has
identified. Therefore, it is necessary to increase
the chances at the better lifestyle where
happiness is the primary goal rather than other
worldly prospects (McHugh Power & et.al.,
2019). It will help her in recovering much
quickly because the mental stress and the state
of depression is the major reason behind the
decreasing health of patient currently.

Asking this question is very important as this
helps in ascertaining what is the wish or desire
of the patient. Many times patients without any
serious diseases or problem wish to undertake
intensive care where they are kept laced in
medical equipments. This is mainly done in
order to seek attention of their loved ones.
