What should be recorded in the document?
· You need to keep a record of your duties and experiences at work
· Evidence from you that reflects your personal assessment of progress in your job
· A record of specific learning opportunities/training/visits that you experience
through work (e.g. an opportunity to follow a patient/client/service user to a
hospital appointment)

· Meetings between you and your mentor/line manager/supervisor and lecturer and
any other people involved in your work.
