INTRODUCTION
Person centered care is a way of thinking and doing things for people by providing the
social and health care services as equal partners in planning, developing and monitoring care to
make sure it meets their needs and wants. In other words, putting people and their families on the
position to make decisions on health services. Person centered is not just given services to people
it is about fulfilling and respect their values, desires, situation and understand their social
circumstances. Present study will be base on the reflective approaches in implementing person
centered practices. Therefore, the report will further outline the different concepts and theories.
Also, this covers the different more topic like comparing social and medical models for person
centered in health practices. Also it defines the different concepts and making the study more
productive towards the aim of the study. Study also emphasizes the current policies and
legislation in the context of effective person centers practices.

Discussing the different legislation are reflected in provision of person centred care.
As according to the act 2008 health and social care and regulations 2014. The main
intention of this regulation process is to ensure that people using a service have care or treatment
that is personalized specifically for them. The main purpose of this regulation is to make sure
that every person take equal care or not (Nelson and Staggers, 2016). All the patient or people
should get the best quality of services and make sure the things more effectively done in efficient
manner as per the need or demand of the patient health. However, it helps to ensure the best
development thing to make the things more effectively achieve and efficient. Also under this
regulation providers must make sure that, weather this position has discovered the effective
working out goals to ensure the best services to the patient. Also, provider has to make sure that
the things has to be solved in an efficient manner.
Under the Regulation 9 the main intention of the care taker is to give personalised
treatment to the patient as per the condition and according to the requirement (Norwic and Eaton,
2015). Providers must work in partnership with the person which makes any reasonable
adjustment and provide support to help them understand and make informed decisions about

their care and treatment options. There are some statutory duties which covers under the Person-
centred care are given below:
Health and social care act 2012 : The aim of this act is to prevent and promote the
involvement of each patient to protect their interest and give them the best treatment growth.
Care Act 2014 : Requires local authorities to involve adults in their assessment in order
to support planning and review. This better understand the objective and aim to make sure the
things in an effective and efficient manner (Stanhope and et.al., 2015). Informing and engaging
the patient for the better achievement.
Integrated care licence condition : This statutory obligation provides authority to the
people to work as a care taker. Also, this regulation give right to care taker providers to conduct
his duties.
CQC : health and social care act 2008 Regulation 2014: Regulations 9 describes the
action to make sure that each person receives appropriate person centred care and treatment.
There are some principles which should be considered by the care taker providers
 Care is personalised: In this the health care unit is involved in giving person cantered
and holistic care to patients. They make use of open communication.
 Care is Coordinated: For coordination aspect health care professionals are being
engaged in keeping coordination with their family members too, in this they make use of
open communication.
 Care is enabling : This is the another foremost principle under the study of making the
study more comfortably effective and challenging.
Suggesting a appropriate solutions to different problems occurred while implementing a
regulations and policies.
Good practices of person centred care can help to protect the social cause and better
provides the services to the patient. Besides, to get the good outcomes and growth providers
must have consider the required progress regulatory considerable principles and laws. This must
have required to progress the high demanding development targets (Yardley and et.al., 2015). By
applying the effective and productive practices of person centric approach this will more helpful
and conformable in the context of the business growth results in the detailed manner. Apart from
that, it also helps to discover the challenging in health care task to fulfil the higher growth.

In order to prevent the patient and improve quality providers must have followed the
proper regulatory system and proper implementing the regulatory policies and growth. Providers
should can also have the quality of work and maintained the proper disciple in order to
implement the proper regulatory system and approach. Besides, acceptance that better access to
care without attention to its quality will not lead to desired population health outcomes. It better
helps to get the highest business growth in case of such effective active performance in the best
quality and growth. Moreover, this business case related to the business system to protect the
care of the services and growth. Social model of person centric is caused by the way society is
organised rather than by a person impairment or difference. It looks at ways of removing barriers
that restrict life choices for the impairment people. In contrary, medical models of person centred
says that people get damaged because of their loss or differences. So, this is the first difference
between social and medical models which is based on two different concepts.
As according to the medical models, the person impairment can be solved by the medical
cure or can be fixed by treatment. In contrary, social model says that disabled person cannot
recover so quickly. It takes time and can affect the person physical mental sensory. This is
another difference that be found between social and medical models of person centred. In within
healthcare planning or implementing process there should be a proper legislation within the
system in order to implement the study and makes the good out goals. However, in person
cantered approach it has to be according to the need of service provider. Legislation and
regulatory system help to protect the uncertainties within the purpose and make better forming
and developing manner.
Challenges faced by health care professionals in interpreting different legislation in relation to
planning.
Ethical code of conduct is the major issue while implementing the health care legislation
and practices. Such challenges that faced by the care professional are cost issue which is the
common issue. Due to the expensive medicines and lack of resources, care providers cannot
maintained the high safety precautions and tools for the care users. This makes the system
improper and create challenges for the care professionals.
Political challenges is the another challenge what might have facing care professional.
Due to changing political parties they have changes the rules and code of conduct accordingly
such changes might have created the issue for care professionals (Yardley and et.al., 2015). Such

business objectives and learning goals might have creating the goals and services as according to
the legal regulations. This business goals have maintained the proper justification and create the
goals and services accordingly in the high substantiation manner. In other words, regulatory
challenges drive up the cost of providing the care services to the patient. Service care taker also
might have facing issues in the context of legal formalities which is necessary to be followed in
systematic manner.
Technology is another major challenge that might have facing by the care professional
while implementing the legal regulatory system into the system. Besides, that it also maintained
the sustainability in the system in order to get the fast and efficient outcomes. Besides, it is very
necessary for the care providers to amend the all latest technologies and new system which
support the legal aspect of the system. It also helps to better services and makes the good impact
on the health care service people. Sometimes new technologies and new version software could
not understandable by the hand of service provider due to which nurses and care taker would
have face challenges while implementing the care services. Technology implications are the
most dynamic challenges that been facing by the employees in the process of implementing the
service measures.
Other challenges that have facing by the care professionals which is patient
confidentiality growth and challenges in different opportunity and perspective and growth.
Besides, due to hiding any information from the care providers can create a problem for the
providers (Yardley and et.al., 2015). Besides, it helps to maintained the good services in the
effective working manner. Other challenges that might have facing service providers like
inefficient sources and knowledge in the specified field and also facing issues with the risky
cases due to which possibilities of uncertainties can be occurred.
Critically evaluate the effectiveness meeting the legislative requirement.
Safeguarding is the process or activity which protecting the life of people who does not
safe their own life. Such kind of people are vulnerable and inefficient. The main purpose of
safeguarding is to maintained the effective culture and maintained the safe environment for the
people to protect the systematic growth and challenge in within the purpose of goals. Meeting the
needs of regulatory system is the growth and challenge for the purpose to protect the system and
emerging market growth. By not adhering to legislatures, laws and rules implemented by central
authority then this might lead to putting a negative image in mind of patients. If laws, rules and

regulations are not followed then health care unit can face huge criticism. This can hamper their
goodwill. Also patients might think before visiting health care unit.

Scenario 1: Give a brief description of the patient or services user or individual in need
that you have provided person center to:
Being a Domiciliary worker, I have recently deal with two cases. One of the cases is on
Alzheimer's patient. He is person of 60 years old live in Australia alone. His wife left him at the
age of 50 from last 10 years he is all alone. Due to frustration and anxiety he become angry
man. All such situations or conditions affect his mental health. When I met him, he is a person
who suffering from Alzheimer’s. I identified the situation and evaluated that the reason behind
the mental health issue is because of loneliness. Being as a care giver, I have done several
things and use different intervention of person centric that helps me in the process throughout.
Along with that, I have used technique or Person centric approach which is VIPS which stands
for Valued, Individuals, Perspective and Social. All such elements together form a good
support for the person who suffers from Dementia. Valued define that patient should get valued
from his care taker while giving services. As I do throughout my session. Reason, Alzheimer's
patient is very sensitive and emotional. He requires special attention from their care giver. So I
always makes sure that John will never get alone and think negative thoughts. Thirds is
perspective of the person with dementia must inform our understanding which is also very
much necessary for patient as well as for mine. Last social aspect of this intervention wants to
describe the positive surroundings and relationship with patient. VIPS is the approach of
Person-centred care which helps me to treat John.
Another approach that I have been used during the process is Palliative Care which is another
significant for John to give quality care. This intervention mainly focusses on three aspect first
is life is affirmed, Distressing symptoms of whatever sort should be actively treated whilst
maintaining quality of life and Holistic care. In this intervention I used to handle John with
holistic approach in which I have prepared sessions with Johns friends, neighbours which
further give happiness to John. Holistic approach is the intervention which gives social and
emotional support to the patient.

Key Principles of Person-Centered care
Affording people dignity, compassion and respect: This principle of person-centred care
support me to maintained the good relationship with John. I never performed any rude
behaviour with John which make him nervous and depress I always listen and understand their
feelings.
Offering Coordinated care: while during the health care session I used to help him in the
activity in which they have performed. I will support him mentally and physically in throughout
the process.
Offering Personalised care and support: In offering personalised care so I used to make
health plan and routine plan for John during the whole sessions.
Scenario 2: Give a brief description of the patient or services user or individual in need

As with my second case with a lady of 55 years old. She is suffering from diabetes and live with
their family in London. Despite that, when I met her she I analysed that she is very energetic
and influencing towards making things a kind of energetic and innovative person. I spend a
good time with her during her treatment. I really enjoyed and facing less issues by treat her in
the duration of time. I have been using different intervention style throughout the process.
Which helps me out in the duration of time. After that I have been used different interventions
for Marry to make her more stable and help her in the best possible manner and growth. I have
used model or approach The Chronic Care Model this is the model which helps to use with the
patient of chronic disease. I have been used different approaches and task for this goals in order
to meet my own aims and purposes with the case. The basic and main purpose of this
framework is to enhancing care delivery by identifying essential components of the health care
system that can be modified to support high quality, patient centred chronic disease
management. Along with that, it also makes the things more clearly mentioned and make
different in the situation. Besides, this model helps me out in different context. Which certainly
based on the issues. While implementing this issue and system I will make sure the following
activities. Also I visibly support improvement at all level of the organisation. I will make her
systematic and make other person. Mobilize community resources to meet needs of patients.

Affording people dignity, compassion and respect: During the process of treatment in the
taking care of her I have been always consider her feeling first and try her to understand in
better way. I always give her respect and support in their difficult process which makes things
more effective and respectful.
Offering Coordinated care : Diabetes is chronic disease which makes people suffer from
different aspect. On the other sides, it is less effective and low generating the work out goals in
different form. During the whole process I help her out from the situations and made different
following results. Also I support her in their daily struggles to take medicines.
Offering Personalised care and support : Being her care taker I have give her several
personalised care that helps her to adjust and stay happy with the issues. In other words, I
provide her to decide what she should eat and what should not. Also provide guidance and
suggestions on their daily routine activities and also make her convenience for exercises.
Reflective statement on both scenario
From the above two scenario I have learn so many things and get a good experience from the
two cases. I have gained knowledge about the principles and practices which should need to
understand and make the things more appropriate. In other words, I explore the assessment of
pain as conducted with one 60 years old man and one 55 years old lady. I tried to give them
support in their respective issues. I implement Person centered principles and also applied.
Along with that, it also helps to give matter and formation with effective duration of time with
the help of different intervention process. In order to reflect this statement I have been using the
Gibbs model that helped me out to present my feelings and study that I have conducted with
two people. It helped me to promote what extent I as a nurse should recommend analysis. I had
visited with two different people with different issues. I found how people and person have
gained the different aspect to performs and manage. I have used provisions and my work
principles that helps me to promote in my career development task and also helps to understand
the different business aspect of the study in different manner.

CONCLUSION
From the above study it has been concluded that how the medical and social models
apply yo person centred practices in health care or support services. Also, the study presented the
deep understanding in the person centred approach when planning and implementing a
programmer for individuals requiring support. Therefore, study also identified the different
legislation and practices required by the nurse and how to implement it with the case. Study also
have covered the topic of implementing specifics regulations and policies in a health and care
settings. On the same side, it also concluded in the study about the reflected statement of the
researcher by supporting and developing personal development plan in an efficient manner. The
purpose of this overall study is to analyse the deep understanding between the aspects which
better analyse the situation and following business goals. The purpose of the whole study is to
explored the understanding in the field of service provider and also affect the goals in indirectly
manner. Apart from this it also might affect the overall goals and make the system
unchallenging. Study concluded the importance of legal challenges.
