Name: Kim Sweeney
Position: Manager Job Title: Operations general manager
2. DETAILS OF INCIDENT
Date: To be mentioned Time: To be mentioned
Location: Commercial Office, Max Lionel Realty
Describe what happened and how:
A client wanted to leave through the emergency exit and fell down fracturing her
wrists. Stacked paper blocked the exit way which prevented easy escape and her
from access to medical aid. An agent helped her but ended up insulting her for her
fashion sense. The customer was taken to hospital but is threatening to sue the
company holding it responsible for her injury and humiliation. An agent smelt a
strange smell but made a verbal reporting of the matter instead making a written
complaint. The HR manager had come to know about the incident and has been
communicated by the lawyer of the customer and the Human Rights Ombudsman
Office, Australia, all of which made the matter very grave. The manager did not
fulfil his responsibilities like maintaining work, health and safety (WHS) records
