Executive summary
The purpose of this report is to analyze the impact of the workplace environment on health of
workers. In this regard, various components at the workplace are addressed including the
purpose of the visit, intended actions for preparation of visit, prohibition notice and actions taken
when a person would have died in incident. It is discovered that the initial visit shall be done
purposefully to investigate the location and workplace and analyze whether management at the
workplace is according to the prescribed rules and regulations. There are different actions like
documents, equipment, and workforce that can be used to investigate the workplace. The action
taken in case of death due to fall includes preparation, site visit, interviews, examination of
evidence, and preparation of final report. It can be concluded that workforce Health and safety is
of major significance to the organization. This is because the organization could get involved in
the legal case if the policies and procedures of employee safety are not complied or followed at
the workplace. It is recommended that a workplace risk assessment must be conducted in order
to analyze the root causes of any incident and prevent them from occurring in future.

Introduction
The aim of this report is to evaluate the impacts of workplace stress on human health. In this
context, the report discusses the visit to the place of the accident. The report determines purposes
behind the visit, actions are taken for preparing for the visit and items which will be taken while
visiting the accident site. The report is also discussing actions taken in relation to the case study
of stress at the workplace. In addition, a prohibition notice for discontinuing the system of work
is also presented in the report.

Task 1: Motive of site inspection
The first visit of a site requires environmental health expert to investigate the location and
workplace and analyze whether management at the workplace is according to the prescribed
rules and regulations. If the company failed to imply on the prescribed norms and regulations
given under law, the inspector is authorized to take lawful action against it. Also, initial visit
includes checking for health hazard and analyzing whether measures of safety precautions for
labour are being used at the workplace or not (King, et al., 2018). Inspection of food, workplace,
public health, an epidemic-prone area in the vicinity, accident investigation, all these issues are
taken into consideration by the environmental expert. While starting a company you should
require a lot of environmental clearances which are described in the books of laws. On the basis
of these, a company gets the message of authorization. If the company fails to comply with the
laws then strict action taken against the company. Initial visits require assessment of risk
mitigation machinery being developed or not, protection from man-made incidents been taken
care or not, and pollution norms been followed or not. Before visiting the place of the
investigation some of the actions are required to be taken. Firstly, it is significant to have a
permit to visit and investigate at any workplace. If you need to investigate any workplace you
have to take a proper permit and warrant issued by the government. Secondly, adequate
knowledge of the company shall be gathered, which will be related to the difference. Moreover,
it also identifies the cordial relation with the sub-departments of the government, which helps the
company in terms of business operations. This helps to reduce any miscommunication and
barrier among the different departments. The inspection officer needs to identify the different
divisions of a company before inspection like health department, agricultural ministry,
municipalities, and business communities so that there can be no issues will arise between the
ministries itself (Shammi, et al., 2017). Record of the past history of the company and the
individual is to be studied carefully and to analyze whether the company or the person is a
willful defaulter or not and whether the company has cleared its bank statements as well as its
police records. The injured person’s professional status is to be evaluated thoroughly to know his
way of working and any lacking in his work quality. Various studies that are to be taken care off
before going for the investigation like equipments, workplace’s risk assessment, meeting with
expert who can potentially the hazardous condition at the workplace, investigation permits, past

record of the company to identify any malicious activity done by the company, root-cause
analysis, internal and external audits, fire safety and fire mitigation machinery in place or not.
Fulfilling the requirement of manpower to carry out the investigation and hiring of
knowledgeable persons for getting the job done. Sub-division of work and prioritizing each work
accordingly for the deserving persons is the job to be done before going for the investigation
(Yang, 2017).
Task 2: Evaluation of actions
There are different actions needed in context of the given situation i.e. first to understand the
whole scenario in which this incident happened, the investigation should be done with 9 other
workers employed there to have a myopic idea of what had happened. It is important to search
the place where the incident occurred and evidence shall be collected for further investigating the
case. Oral explanations of the incident should be taken from the co-workers, the site supervisor
and the managing director of the company, Jonathan Jones. Investigation of the forklift truck
should be done to know if it was carrying overload and because of this incident happened. Oral
explanations from the injured party are taken if he is in the condition to do so (Jia, Rowlinson,
Ciccarelli, 2016). Check for the insurance of injured party as prescribed by the government that
it is mandatory for every organization to have insurance of the workers. If the injured party is not
aware of this then help him or his family to get the claims. Powers of the investigation officers
include enforcement of the laws and legalities involved in such a process. The order of the court
shall be enforced so that the convicted person can be put behind the bars. Maintenance of the
public registers of notices is an obligation and mandatory on part of the officer and its council
under environment safety act 1988 (Lavery, 2013). Breach of law should be dealt with radical
measures as prescribed by the law and enforced by the officer. For maintaining the
confidentiality of the individual, data protection act 1998 is enforced (Webster, 2017). With
regard to the situation mentioned, use of stillage was not mentioned in the company risk
assessment, although it mentioned the use of lifting pallets and not the use of stillage. This is
opposite to that of company risk assessment hence the company falls in the category of defaulter
with regard to false information provided by them. Under these circumstances, the company is
liable to pay compensation to the injured party and is also eligible to pay for the medical bills as
appropriated by the hospital. Stillage was not being secured from the site of the incident and this
is why it slipped off. This suggests that the company is in fault and strict actions shall be taken

against it. Whatever is prescribed by the government if not been followed will lead to a default
on part of the company and require immediate action by the officer against them.

There are several steps to do the investigation of a person who dies due to falling. These steps
are discussed in detail below:

Preparation
A plan for investigation shall be developed which includes the determination of techniques
which will be used for investigation, investigation sequence, data which will be analyzed, the
involvement of individuals, and the expected outcomes. Moreover, the resources are required for
the investigation such as budget, time, equipment, and staff shall be taken into consideration
(Reason, 2016).
Scene visit
Firstly, it is important to take the situation under control by shutting down the operations and
securing the area. The people will be taken out of the incident area and management shall be
notified that the area will not be accessible for common people until the investigation is
completed. Secondly, the appropriate people will be contacted and the information from the
eyewitnesses will be gathered. Thirdly, the place of incident will be sketched and important
condition such as position of the died person, and equipment involved at the time of the incident
will be taken into consideration. Apart from this, photographs shall be taken and video recording
will be done to analyze the condition of the site in an effective manner (Neuendorf, 2016).
Moreover, samples and evidence will be collected such as the stillage and fork lift trucks which
are suspected of malfunction, failure or faulty design. Lastly, the site will be returned to normal
use after all the relevant information is collected.
Interviews
Interview is a significant tool for gathering information from the relevant people associated with
the incident of fall. The persons who will be interviewed will include the supervisor on whose
instruction the work was being processed, trainer of the worker who died, the other worker who
perform the same job, eyewitnesses who saw the incident in actual, people who came just after
the incident, and also those people who have information related to work tasks, use of materials
and equipments, process of the company, safety devices in use, and other conditions involved in
the incident (Santos, 2016). The information gathered from different people will be documented
or taped so that it can be used further for drawing conclusions.
Examination of physical evidence
In order to examine the evidence, it will be sent to the expert for further analysis. The expert may
be a health professional or engineer or manufacturer. The other documents such as the report of
past incidents, maintenance reports, minutes of health and safety community, and training

records will also be examined to identify the root cause of the incident and determine whether
the policies and procedures of workplace safety comply or not (Menon, 2017).
Report preparation
The last step is to prepare the final report which includes the incident details. It will also include
the corrective measures which will be taken to improve the health and safety at the workplace
along with the target dates of completion. A copy of the report will be kept with the Workplace
Health and Safety Committee.

Conclusion
On the basis of the above report, it can be concluded that Health and safety of the employees at
the workplace is a major significance to the organization. It is because the organization could get
involved in the legal case if the policies and procedures of employee safety are not complied or
followed in proper manner at the workplace. In case of any incident, a proper plan for
investigation is to be followed which includes preparation, site visit, interviews, examination of
evidence, and preparation of final report.
