Describe what happened and how: The incidence occurred on Wednesday when one of the
clients fell and broke her wrist. The client had decided to use the emergency door since the
exit door was already blocked by the boxes of paper. The client got helped by one of the
agents who handled the exit door after hearing the call for help. The agent had no idea of
what had happened and ended up criticizing the client's high heels. This made the client feel
humiliated and felt a need to sue the company. The client was later taken to women's
hospital by the Ambulance.

Describe what happened and how
The client fell and broke the wrist. The agent who went to help the client did not know what had
happened and criticized the client's high heel. This led to the humiliation of the client resulting

The incidence occurred due to the following factors the ignorance. The member of staff who
noted the oily substance on the stairwell could inform the manager inform of written
communication.
Poor communication. The management failed to identify the possible risks which could help in
taking appropriate actions to prevent any accident for both employees and the customers.
This is in accordance to the Australian Hardware risk management plan.
It states that the essential indicators for any risk should be pointed out, monitored closely and
the action to be taken where possible by all the members of staff.
