Recording of Evidence in Dental Care: Methods and Importance

This document provides information on the methods and importance of recording evidence in dental care. It discusses the different types of dental records and their usage, ways to monitor physical characteristics, and support patients during oral assessment. Purpose: to record learner evidence against a range of assessment methods
The Evidence Record needs to include a clear description of the evidence being presented to enable
the assessor to make appropriate assessment judgements.
This document can be used to record a range of assessment methods. For direct observations and
professional discussions, the Evidence Record needs to be completed by the tutor/assessor.
However, the learner can complete the Evidence Records for other evidence such as reflective
accounts or learner plans and records, the document can also be completed by Expert Witnesses to
record their statements.
Feedback in relation to the assessment recorded on this paperwork will be given on the assessment
cycle feedback sheet.

The tutor/assessor should indicate which assessment method has been
used by adding the appropriate assessment method from the recommended
list for the qualification.
A list of recommended assessment methods can be found in Section 2 of
‘How to …’ A Guide to Assessing CACHE Qualifications, which is
available on cachezone.

This section should provide a clear description of the learner’s practice to
enable the tutor/assessor to make their assessment judgements. It can be
completed by the tutor/assessor, the learner or the Expert Witness
dependent on the nature of the evidence.

1) Methods to monitor the physical characteristics
Physical characteristics of an individual can be monitored by various
ways such as:
 Heart beat / Minute
 Breathing rate
 Pulse rate
2) How to alert a clinician about a potential medical emergency
The clinician can be informed about a medical emergency by the
means of telephonic call. Text message or referring the medical reports of
the patients (Treloar and et.al., 2015). This will help him top make suitable
preparation to address the emergency.
3) Way to support patient during oral assessment
A patient can be carried out with proper assessment and
management of the treatment that is been carried out within an organisation
which will help in improving his health condition that will improve the
quality of services that are been performed within the organisation in much
suitable way (Ritter, 2017.).

1) Internal practice referral system
It is a practice that will help in the proper management of medical
emergency by a doctor or dental practitioner. This will help in effectively
handling a serious condition or medical attention to a patient internally a
department or clinic.

The need of further assessment will help in proper management of
treatment operations will help in reducing the severity of ailment. This can
be communicated either face to face or by producing a patient referral
document. It is important for having a proper treatment of dental disease.
4) Surgery procedure for further treatment
For the further treatment, Flap surgery or pocket reduction surgery
measure can be taken in consideration by the dental practitioner to treat a
severe dental ailment.
CONCLUSION
On the basis of evaluation, this can be said that the proper support
and help is required to carry out the effective dental assessment to a greater
extent.
