Purpose: to record learner evidence against a range of assessment methodsThe Evidence Record needs to include a clear description of the evidence being presented to enable the assessor to make appropriate assessment judgements.This document can be used to record a range of assessment methods. For direct observations and professional discussions, the Evidence Record needs to be completed by the tutor/assessor. However, the learner can complete the Evidence Records for other evidence such as reflective accounts or learner plans and records, the document can also be completed by Expert Witnesses to record their statements.Feedback in relation to the assessment recorded on this paperwork will be given on the assessment cycle feedback sheet.Learner / assessor detailsGeneral information to identify the learner and tutor/assessor. Assessment method The tutor/assessor should indicate which assessment method has been used by adding the appropriate assessment method from the recommended list for the qualification. A list of recommended assessment methods can be found in Section 2 of ‘How to ...’ A Guide to Assessing CACHE Qualifications, which is available on cachezone.Recording of evidence This section should provide a clear description of the learner’s practice to enable the tutor/assessor to make their assessment judgements. Strategies to overcome the oral disorder can be communicated verbally and nurses can make use of researched based strategies. Effective communication will help in overcoming disorder (Slade and Spencer, 2014). Resources which is been required to promote oral health is global channels to reach wide audience3. Respect of personal beliefPersonal belief can be respected by like for example Understanding the cultural needs and demands of patients.Not asking any personal question which can hurt there sentiments.Respect must be given to thoughts and values of patients.Taking care of religious belief of service users.4. Ethnic and social background of patientVarious things can be taken from patient when promoting for oral health. Internal practice referral systemNurses can ask for referral when they are surrounded with insufficient resources to promote oral health. It can be taken from any certified dentist. For example dentist can be engaged in direct verbal communication with nurses to provide them with sufficient resources to overcome the disease or disorder. Intensive public communication and education is essential to inform the public how, where and when they should seek health care at different levels. 6. Understanding of informationWay to provide information that is accurate and consistent with organisational guidelines are if individuals are older than written data with facts and pictures can be given by healthcare professional to them.
