3. Questions/Task
Read the scenario and then answer the questions that follow.
You work at CareShore as a Case Manager.
As part of your role as a case management supervisor, you are required to review case management
plans and practices to ensure that practice standards and legislative requirements are adhered to.
As part of your review processes, you must also evaluate and improve your own performance. This
includes staying up to date with trends in the industry.
Part 1
A. Complete the following self-assessment. When answering the questions, you may reflect on your performance
during this course, within the case studies/scenarios and within your structured workplace learning. You should
complete this self-assessment in conjunction with your peers or supervisor where possible. If you are not able to
do this with someone in the workplace, complete it with another student, a friend or family member.

Three of my strengths are empathy, adherence to professional boundaries and commitment to nursing and
meeting clients needs. I can well interact with the patients by displaying variety of emotions and help them cope
up with physical and psychological issues. Working within professional standards allows maintain patient’s
dignity, autonomy and respect. My committed makes me responsible in delivering the health care.
Based on this assessment, list and describe three strategies you can implement to improve your professional
performance? (50-75 words)
To improve my performance the three strategies are-
To consult the senior nurses for decision making and always seek specialist advice. It will help
know better strategies from peers
Engage in self directed learning through online modules for quality improvement
I will invest more time in communicating with the client to know the needs and preferences
B. How can your personal beliefs, value and behaviour affect your professional practice? (50-100 words)
My personal values, beliefs and the behaviour can affect the professional practice as because the NMC states the
nurses to provide anti-discriminatory health care. However, there are issues of accountability, negligence and
responsibility to be handled as the patient’s values and beliefs may not align with the personal values and beliefs.
It is the main issue when working in culturally diverse setting. Further these beliefs and behaviour may affect the
professional practice owing to the code of ethics ().
C. Describe two (2) models of professional reflection that you could use and their processes.

Bowers Reflective Model
in this process one has to know why is reflection is important,
followed by reflection on the occurrence of the event. The event is
then analysed by existing literature. The learning from the event is
then used to improve the situation in future. Lastly, an action plan
is developed to handle the situation in current context (Roulston,
2010)
Atkin’s and Murphy problems The process of reflection begins with describing the situation and feelings
associated with it followed by analysis of the feelings to explore the
alternatives. The existing knowledge is evaluated an identified any
learning that occurred in this process. Lastly, an action is taken for new
experience (Moon, 2013).

health records due to increase data
demand has been a new trend in
the health care industry
Nurses can adopt the EHR for smooth workflow while avoiding
issues with running notes. It will help health practioners to better
make clinical decisions. Nurses can track the physician’s clinical
protocol and access patient’s chart anytime. It is helpful for
nurses as they handle 75% of the chart work (Silow-Carroll
Edwards & Rodin, 2012)
Patient centered care to increase
patient satisfaction and
engagement. It involves putting
patient at the centre of care
The nurses can engage with patient beyond the traditional
hospital visit by fulfilling their needs effectively. The nurse can
enhance the inter-professional coordination of care. The nurse
may assist the patient with usage of smart phones, online
shopping, and automatic banking for shared decision making with
patients (Epstein & Street, 2011)
B. Learning is a vital process within professional practice. Briefly describe each of the different learning styles and
how they impact learning.

Visual it is the style of learning using the charts,
maps, graphs and images to stimulate
the visual cortex of brain for encoding
the information. This process leads to
learning and is known as visual learning
(Gilakjani, 2011)
I will ask the learner to write down what
is taught, prepare graphs and diagrams,
use symbols instead of words and help
create the mind map of the ideas. The
class may involve preparation of poster
on the chosen topic to help learning by
visualising the mechanism of physiologic
pathways
Kinaesthetic is the style of learning involving physical
activities for manipulating the touch
material to learn in combination with the
auditory and visual study techniques.
Therefore, it is also known as Kinesthetic-
Tactile Style as it involves multi- sensory
learning (Gilakjani, 2011)
I will help the learner by developing case
studies to stimulate the critical thinking,
improve clinical problem solving and
communication skills. Other strategies
may include use of videos and creation of
timelines to explain the concepts during
the cardiac lecture. Student can be on
treadmill while listening to audio records
of lectures to better help understand the
cardiac function.
Social It is the learning style involving verbal
and non verbal communication with
people demonstrating strong social style,
where learning occurs by involving in
social activities. The learning style is
defined by interpersonal; communication
(Rosenthal & Zimmerman, 2014)
I will help the nurses with rewards for
positive reinforcements. Learning can be
initiated by role modelling and
observational earning as mentioned by
Bandura’s social learning theory. It will
help the nurse to observe and learn while
the learner is able to determine the
